Chagas disease as a risk factor for congestive heart failure: Based on a hospital study, Nicaragua 2003

Abstract

The objective of this study is to determine if Chagas disease is a risk factor for congestive heart failure in patients admitted to the Internal Medicine Department of the Dr. Oscar Danilo Rosales Argüello Teaching Hospital (HEODRA) in the city of León. The research was conducted between April 2002 and October 2003. A hospital-based, unmatched case-control study was conducted at HEODRA. A total of 151 cases of congestive heart failure (CHF) were studied, each meeting at least one major and two minor criteria according to Framingham criteria. For each case, a control (151) without congestive heart failure was selected. Odds ratio and 95% confidence interval were calculated. Multivariate logistic regression analysis was performed to control for potential confounding factors. The main findings were that 24.5% of the CHF patients admitted to the Cardiology Service of HEODRA tested positive for Chagas serology, compared to 11.9% of controls with positive Chagas serology (OR: 2.39, 95% CI: 1.29–4.44), which was statistically significant. In the multivariate model, the variables that influenced the development of CHF were age, sex, and positive Chagas serology. In conclusion, Chagas disease is more than twice as associated with congestive heart failure, as well as other associated factors such as age >51 years and female sex, which were statistically significant in this study.
