ABSTRACT
Tolosa-Hunt syndrome is a rare clinical entity with controversial etiology and pathophysiological mechanisms. It is characterized by headache associated with paralysis of one or more cranial nerves, resulting in diplopia, strabismus, and ptosis, due to involvement of the cavernous sinus or superior orbital fissure. Diagnosis remains challenging and is considered within the differential diagnoses of painful ophthalmoplegia. In order to describe and update current knowledge about this condition, we present the case of a 14-year-old female patient who presented with severe headache, ocular pain, and cranial nerve involvement. Clinical findings and magnetic resonance imaging confirmed the diagnosis of Tolosa-Hunt syndrome.
