Testicular microlithiasis: case report

Abstract

Introduction: Testicular microlithiasis is an uncommon condition that is often incidentally diagnosed by testicular ultrasound. Diagnosis is essential for providing adequate follow-up considering the possible conditions associated with TM. Case description: A 3-year-old male patient with a history of inguinal hernia and left cryptorchidism. Prior to orchiopexy, a Doppler testicular ultrasound was performed, confirming left cryptorchidism and reporting both testicles without alterations and no microcalcifications. Orchiopexy and hernioplasty were performed at 14 months of age with a good postoperative evolution. During a follow-up visit for a respiratory condition at 2 years of age, a right inguinal hernia was detected with a silk friction sensation at the right inguinal level and both testicles with good scrotal localization and no evidence of masses or other alterations on physical examination. Prior to right hernioplasty, a new inguinal and bilateral testicular ultrasound was requested, which showed the presence of a bilateral diffuse starry sky pattern compatible with testicular microlithiasis. Conclusions: This case highlights the importance of routine check-ups, as testicular microlithiasis was not identified in the previous ultrasound and could have remained undiagnosed if not for a regular and thorough evaluation.
